Indications/Uses
HSV infections of the skin & mucous membranes including initial & recurrent genital herpes. Suppression (prevention of recurrences) of recurrent herpes simplex infections in immune-competent patients. Prophylaxis of herpes simplex infections in immune-compromised patients. Varicella (chickenpox) & herpes zoster (shingles) infections.
Dosage/Direction for Use
Adult Herpes simplex infections 200 mg 5 times daily 4 hrly w/o night dose. Continue treatment for 5 days, but may be extended in severe initial infections. Severely immunocompromised patient (eg, after marrow transplant) or those w/ impaired gut absorption Double the dose to 400 mg or alternatively, IV may be considered. Suppression of herpes simplex infections in immunocompetent patient 200 mg 4 times daily 6 hrly or 400 mg twice daily 12 hrly. May titrate dose to 200 mg 3 times daily 8 hrly or 200 mg twice daily 12 hrly. Periodically interrupt therapy at 6-12 mth intervals. Prophylaxis of herpes simplex infections in immunocompromised patient 200 mg 4 times daily 6 hrly. Severely immunocompromised patient (eg, after marrow transplant) or those w/ impaired gut absorption Double the dose to 400 mg or alternatively, IV may be considered. Varicella & herpes zoster infections 800 mg 5 times daily 4 hrly, w/o night dose. Continue treatment for 7 days. Severely immunocompromised patient (eg, after marrow transplant) or those w/ impaired gut absorption Consider IV dosing. Treatment & prophylaxis of herpes simplex infections in immune-compromised patient Childn ≥2 yr same as adult dose, <2 yr ½ the adult dose. Treatment of varicella infections Childn >6 yr 800 mg 4 times daily, 2-6 yr 400 mg 4 times daily, <2 yr 200 mg 4 times daily. Dose may be more accurately calculated as 20 mg/kg (Max: 800 mg) 4 times daily. Continue treatment for 5 days. Renal impairment: Severe (CrCl <10 mL/min) Treatment & prophylaxis of herpes simplex infections 200 mg twice daily 12 hrly; Treatment of varicella & herpes zoster infections 800 mg twice daily 12 hrly; Moderate (CrCl 10-25 mL/min) Treatment of varicella & herpes zoster infections 800 mg 3 times daily 8 hrly.
Contraindications
Hypersensitivity to aciclovir or valaciclovir.
Special Precautions
Ensure to maintain adequate hydration in high doses. Renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Headache, dizziness; nausea, vomiting, diarrhoea, abdominal pain; pruritus, rash (including photosensitivity); fatigue, fever.
Drug Interactions
Increased plasma conc w/ drugs competing the elimination via active renal tubular secretion. Increased AUC & reduced renal clearance by probenecid & cimetidine. Increased plasma AUC of mycophenolate mofetil.
